About Us

For compassionate, comprehensive home care, choose John Paul Home Care. Our dedicated health team consists of nurses, therapists, medical social workers, home health aides, and dietitians. Our agency offers Middle-Eastern nurses and physical therapists to assist with language barriers. Also, we offer Middle-Eastern interpreters to assist our clients during skilled visits. Following the physician’s care plan, we work closely with patients, their families and physicians to provide result-oriented, short-term home health care that promotes independence to speed recovery.

JPHC consistently stays below the state and national average for sending patients to the hospital and in turn avoids costly and debilitating rehospitalizations. Patients benefit from the convenience of care in their home; they are treated in the privacy and security of their homes surrounded by the people and things they love. JPHC caregivers have one priority: results-focused, individualized treatment. You and your loved ones deserve it.

Home health care is a wide range of health care services that are provided in wherever you call home. Home health clinicians work collaboratively with you, your caregivers and your doctor to help you recover from illness, injury or surgery and assist in helping you get back to your previous level of independence. Services could potentially include skilled nursing, physical therapy, occupational therapy, speech therapy, medical social workers and home health aides.

Medicare requires you to be homebound to receive home health services. Being homebound means you have trouble leaving your home without assistance (like using a cane, wheelchair, walker, or crutches; special transportation; or help from another person) because of an illness or injury OR leaving your home isn’t recommended because of your condition OR you’re normally unable to leave your home because it’s a taxing effort. Also, a doctor or other authorized health care provider must have a face to face encounter with you prior to certifying that you need home health services.

Medicare, like most other insurance plans, does not cover the following: 24-hour-a-day care at your home, meal delivery to your home, and chore worker or non-skilled services, including custodial or personal care. For exact coverage, contact the customer service number on the back of your insurance card.

Home health services cost $0 out of pocket for approved disciplines under Medicare Part A. With Medicare and any other insurance, the home health agency should tell you if any items or services they provide aren’t covered, and how much you’ll have to pay for them. This should be explained verbally to you and in writing via an Advance Beneficiary Notice (ABN).

The quickest way to get services is to contact your primary care provider and have them fax your information and an order to a home health agency that is in network with your insurance plan. If they are unfamiliar with home health, have them contact the home health agency of your choice and ask for their referral department.

LiveWell Partners is a network of home health care providers anchored by a deep commitment to exceptional care.
