Home health care is a wide range of health care services that are provided in wherever you call home. Home health clinicians work collaboratively with you, your caregivers and your doctor to help you recover from illness, injury or surgery and assist in helping you get back to your previous level of independence. Services could potentially include skilled nursing, physical therapy, occupational therapy, speech therapy, medical social workers and home health aides.

Medicare requires you to be homebound to receive home health services. Being homebound means you have trouble leaving your home without assistance (like using a cane, wheelchair, walker, or crutches; special transportation; or help from another person) because of an illness or injury OR leaving your home isn’t recommended because of your condition OR you’re normally unable to leave your home because it’s a taxing effort. Also, a doctor or other authorized health care provider must have a face to face encounter with you prior to certifying that you need home health services.

Medicare, like most other insurance plans, does not cover the following: 24-hour-a-day care at your home, meal delivery to your home, and chore worker or non-skilled services, including custodial or personal care. For exact coverage, contact the customer service number on the back of your insurance card.

Home health services cost $0 out of pocket for approved disciplines under Medicare Part A. With Medicare and any other insurance, the home health agency should tell you if any items or services they provide aren’t covered, and how much you’ll have to pay for them. This should be explained verbally to you and in writing via an Advance Beneficiary Notice (ABN).

The quickest way to get services is to contact your primary care provider and have them fax your information and an order to a home health agency that is in network with your insurance plan. If they are unfamiliar with home health, have them contact the home health agency of your choice and ask for their referral department.

LiveWell Partners is a network of home health care providers anchored by a deep commitment to exceptional care.

At Apollo Medical, we are committed to transparency in the services we provide. In accordance with federal and accrediting body standards, we offer the following information to ensure patients, caregivers, and referral sources clearly understand the scope of our services and any applicable limitations.

Service Availability

Our home health agency provides skilled services under the direction of a physician and in compliance with CMS guidelines and accrediting body policies. Services are available to eligible patients within our licensed service areas. Care is provided in accordance with individual patient needs and plan of care.

Service Limitations

While we strive to meet the needs of each patient referred to our care, the following service limitations currently apply:

  • We do not provide services to patients under the age of 18.
  • Patients who require twice-daily skilled visits may not be eligible if no caregiver is available in the home to provide interim support.
  • Our services are generally limited to a 30-mile radius from each agency office, and only within the counties licensed for that agency.

Please contact our intake department for questions regarding service availability or to verify whether care can be provided in your area.

Public Access to Service Information

This information is reviewed annually or more frequently as services change. It is available to the public via our website and can also be provided upon request by contacting our intake department.
